| THE BRISTOW HOMELESS UNIT (PATH)
The Bristow Homeless PATH unit is designed to serve people of all ages, who are:
homeless,
diagnosed with a major mental illness, AND
not receiving services at another MHMRA Clinic.
Program components include:
Assertive Outreach,
Assessment and Intake, AND
Intensive Case Management.
Clients open to the Bristow Unit are able to access the following clinical and non-clinical services:
physicain assessment,
medication management,
individual and group counseling,
substance abuse assessment and counseling,
nursing services,
wrap around case management,
sack lunches,
showers,
laundry,
phones,
lockers, and
access to the clothing closet.
Referrals to the Bristow PATH Program come from community agencies, other NPC units (PES, CSU, CRU, MCOT) and individuals.
Location: 2627 Caroline Street, Houston, TX 77004.
Hours of Operation: Monday – Friday 8:00 am – 4:30 pm.
