Waiting
List – process by which MHMRA of Harris
County maintains an up-to-date list of individuals waiting to receive
services. Waiting list are kept chronologically by date of request.
Assessment/Eligibility
– process by which a consumer is assessed to determine
his/her eligibility for a program.
Service
Coordination – A system to authorize,
coordinate, and monitor services for a consumer. Service Coordinators
may assist in such services as assessing special needs, authorizing
appropriate services to support the consumer’s goals, and monitoring
the consumer’s to ensure his/her needs are being met.
Person
Directed Plan – process in which the
Inter-Disciplinary Team (IDT) composed of the consumer, family, Legally
Authorized Representative, and Service Coordinator, identify outcomes
the consumer goals he/she wishes to achieve, and the appropriate provider
services to achieve those goals.
Service
Authorization – a process of approval
by the Service Coordinator for the consumer’s Individual Plan of Care
(IPC). The provider then has the responsibility for making sure the
consumer receives supports and services that are outlined in his/her
IPC.
Utilization
Review – a process to determine whether
the services a consumer receives are necessary, cost-effective, and
meet the consumer’s requirements for care.
Provider
Choice/Linkage – a process in which the
consumer/family or Legally Authorized Representative, with assistance
from the Service Coordinator, chooses a provider from where the consumer
will receive services. The Service Coordinator then helps the consumer
to enroll with the chosen provider.
Service
Monitoring – a monthly review process
of the services a consumer receives from the provider to make sure those
services are being delivered according to the consumer’s Individual
Plan of Care (IPC) and assist in achieving personal outcomes. This process
is completed by the Service Coordinator.
Program
Audits – a review of the consumer’s records
to determine the provider’s level of compliance with contract requirements.
This process is completed by MHMRA of Harris County’s Quality Management
Department.
Technical
Assistance – Consultation with provider
staff to give necessary information and training to achieve required
level of compliance with contract requirements.
Survey
& Certification – a process by which
MRLA providers receive a certification review to ensure they are in
compliance with the MRLA Program Principles for Program Providers. Each
MRLA provider will receive a certification review at least annually
in order to maintain certification status.
